Please explain the educational or technical point that this video addresses.: Performance of valve-sparing root replacement with complex coronary anomaly influencing dissection, root preparation, primary suture line for annular stabilization and graft placement.
Please provide a 250 word summary of the surgical video being submitted.: We present a case of valve-sparing root replacement with an anomalous left circumflex arising from the right sinus with a retroaortic course. The patient had an enlarging root aneurysm with moderate aortic insufficiency. We demonstrate key aspects of the preoperative workup including findings of anomalous coronaries on CT, echo, and catheterization, as well as key intraoperative technical points and decision-making for pertinent modifications to the prototypical David-V valve-sparing root replacement with preimplantation technique.

Describe key findings on preoperative CT scan, echocardiography, and catheterization that may indicate anomalous coronary anatomy
Demonstrate technical modifications to the reimplantation technique for valve-sparing root replacement in order to preserve anomalous coronary arteries.
